We will offer two basic fire fighting academies starting in January 2008. These classes will be 450 clock hours and will contain both the FIREFIGHTER I & FIREFIGHTER II curriculum. We will offer a NIGHT class that will begin January 22, 2008 and end in the middle of July 2008. This class will meet Monday thru Thursday from 5 P.M. to 10 P.M. CST. The DAY class will start on January 28, 2008 and end in the middle of May 2008. This class will meet Monday thru Thursday from 8 A.M to 4:30 P.M CST. (THESE DATES ARE SUBJECT TO CHANGE). In order to be considered for these classes, you must meet the minimum qualifications for Fire Standards and Training as established by the Florida State Fire Marshall and qualify to be a student at Chipola College.
You must submit:
1. A completed Chipola College Application
2. A completed Applicant's Information Sheet
3. A copy of your birth certificate, drivers license, social security card and high school diploma.
4. A passing score on the Test of Adult Basic Education (TABE) of 10.0 in reading, math and language. This test is offered here at Chipola College. Call for more information.
5. A completed D14-1020 DIVISION OF STATE FIRE MARSHAL PERSONAL INQUIRY WAIVER Must be signed and notarized.
6. A completed D14-1461 DIVISION OF STATE FIRE MARSHAL TRAINING AFFIDAVIT. Must be signed and notarized.
7. A completed D14-1022 MEDICAL EXAMINATION. DO NOT GET A MEDICAL EXAMINATION UNTIL YOU ARE ACCEPTED INTO THIS CLASS. This Medical Exam must be within 6 months of the first day of class and the form provided must be used. Per FS633.34, as of July 2, 2005 the medical examimation needs to be completed by a physician, surgeon, or physician assistant per ch. 458; or an osteopathic physician, surgeon, pr physician assistant per ch. 459; or an advanced registered nurse practitioner per ch. 464.
All documents (except for the medical examination) should be completed and returned to me as soon as possible. Exceptions are made for the TABE TEST. You may turn in your application without this test, but you should take the test as soon as possible. There is a cap of 24 students for this class.
