Instructions for Completing the
State University Application for Admission

Applying to a university can be a lengthy process. A useful tip is to allow six to nine months prior to the actual deadline for the term in which you plan to transfer. This will allow plenty of processing time and will enable you to be eligible for special provisions offered to A.A. transfer students, such as priority orientation and early registration. Applications are available in Student Services; however, you are encouraged to apply on-line (Click on the university below to access on-line application).

Read the application carefully before you fill it out. Check the university (page 4) to which you are applying. (If you wish to apply to more than one university, a separate application and fee must be submitted to each institution, along with the other required documents.)

  • If you have applied to a university within the last two years, you may be eligible for a reactivation form (no application fee required). To obtain the form, students should contact the university’s admissions office.
     
  • If you have applied to a university and have taken a class, you may never need to pay the fee again. You will have to submit a readmission application (no charge). Students should confirm with the university’s admissions office.

    1. Application for Admission – Attach check or money order made payable to the university for the appropriate application fee. If the university has more than one area campus, enter the name of the campus you plan to attend. Be sure to answer question 15 on page 3; to read, sign and complete statement 21 on page 3; and to complete and sign the Florida Residency Statement on  page 5.
       
    2. Official Transcripts – An official transcript from every school attended is required. A comprehensive Chipola transcript is not a substitute. Transfer students who completed 2 years of the same foreign language in high school must submit official high school transcripts.

      It is the student’s responsibility to request that a Chipola transcript be sent to the university at the end of every semester, and a final transcript be sent to the university, noting completion of the A.A. degree. If you have a "hold" on your records at Chipola, the transcript will not be sent.
       
    3. Admission Test Scores – If you must provide copies of an ACT or SAT test score report and they are not recorded on your official high school or college transcript, contact your high school for instructions on how to obtain copies, or call:

    If you recently took the SAT or ACT and had the scores reported to a Florida institution, your scores can probably be requested electronically. If your name has changed or if you listed an incorrect Social Security number at the time you took the test, it will be necessary to request the scores from the SAT or ACT office.

    2. Mark only one term. Most universities make an allowance for only one term of entry. Use the reactivation form to change that date if it becomes necessary.

    11. The university is subdivided into individual colleges, e.g., College of Education, College of Business. The admissions process is two-tiered. The application you are filling out now is used to admit you to the university. By writing the college and the major on line 11, your admission into one of the colleges is initiated. Admission into the university does not mean automatic admission into one of the "limited access" programs. See a Chipola counselor to identify "limited access" programs, or obtain the university’s catalog. If you are uncertain what to include in this section, talk to a Chipola counselor.

    13. Complete all required information except for the high school code; it is not required of Florida community college A.A. transfer students.
       
    14. Fill this section out clearly and completely. If you need more space, an additional sheet of paper may be attached to the application.

    Be sure to write in the expected or earned dates of your AA degree or any other postsecondary education degrees that you plan to earn or have earned.

    1. Must be answered. This is confidential information, and normally does not have an effect  on an admissions decision.
    1. All students expecting to complete the AA degree must provide official records of their CLAST scores as soon as they are available. Please indicate the date you last attempted the CLAST or the date you plan to take the CLAST.
       

    Students planning to transfer to a university, who have not completed an AA degree and whose native language is not English, are required to submit a TOEFL score.

    2. The first section is to include the courses in which you are currently enrolled (i.e., at the time that you are completing the application).

    The second section should include any coursework you plan to complete before entering the university (i.e., the next semester or two semesters). This section is particularly important for students seeking admission to "limited access" programs. Extra sheets may be attached to the application, if necessary.

    1. Non-U.S. Citizen Only: The type of visa must be noted here. Resident aliens must provide a copy of their resident alien cards (front and back). Staple a copy to the application.
       
    2. Must be completed. No exceptions. Go back two to three years. Be sure to indicate city and state with the applicable dates.
       
    3. Application must be signed in ink; include Social Security number and date. Attach check or money order for the appropriate fee.

    RESIDENCY CLASSIFICATION (Page 5)

    First Section (A-N) – Check the statement that applies to you. (Where prescribed, copies of certificates and other documentation must be sent to the university’s admissions office.)

    3. The claimant is the student, spouse, parent, or legal guardian who is the legal resident of Florida.

    11. If applicable, be sure to copy both sides of your resident alien card. If the claimant is not the student and is a resident alien, both sides of his/her resident alien card must be submitted.

    Non-Florida Residents Only

    IMPORTANT NOTE: If you believe you will attain Florida resident status for tuition-paying purposes by the first day of class for the term for which you have applied, it will be to your advantage to complete the Florida residency affidavit now.

    NOTARIZATION IS NOT REQUIRED.
